ATHLETE APPLICATION

To register a new athlete, the following documents must be completed and sent scanned in PDF format for the documents and JPG for the photo to the club's e-mail ( lynxfc.pentelis@gmail.com ).

  1. New Athlete Application (without doctor's stamp)
  2. Personal Data GDPR
  3. Athlete's Health Card (only from a Cardiologist)
  4. Birth Certificate or Identity Card (>13 years old)
  5. Deposit 10 Euro in the Iban of EOX, using the payment code that will be sent to you by our association
  6. Face photo in jpg format.
